Cross-Connection Survey
The Village of Cullom is implementing an active cross-connection control program as required by the Illinois Environmental Protection Agency (IEPA) and the Illinois Department of Public Health (IDPH). A cross-connection is any situation that allows a potable (safe) water source and a non-potable (unsafe) water source the opportunity to come into contact with each other.
This requires a survey of all customers served by our public water supply. We ask you to complete and return the completed survey to the Village by October 27, 2014. Simply mark all plumbing fixtures you have in your home or business. If you have other fixtures not listed please mark in the ‘other’ spaces.
After we receive your survey we will review the data and determine if an inspection of your plumbing is needed. If it requires an inspection you will be notified. These surveys are required by the IEPA and must be completed. If we don’t receive your completed survey by the October 27th deadline, we will contact you to set a date at which time the water department will conduct the survey.
As a way to thank you for your cooperation, all RESIDENTIAL surveys returned by Monday October 27, 2014 will be eligible for a FIFTY DOLLAR GIFT CERTIFICATE from Cullom Community Market. One completed survey will be drawn at the regular scheduled Board Meeting on October 28th.
Completed surveys can be mailed to Village of Cullom PO Box 166, dropped off at the Village Office 119 N. Oak St Wednesdays 5:00 PM – 7:00 PM or hand delivered to Tim Lovell from the Water Department during the day.
